1、百瑞宠物公园寄养协议 PETZOO BOARDING AGREEMENT亲爱的客户：为了给您的宠物提供一个安全健康的生活环境，请仔细阅读以下内容，并赞同。1.请确认您的爱犬持有上海市政府颁发的“养犬许可证”，并在有效期内。2.请确认您的爱宠已及时接种了足够的和必须的疫苗，我们不对您的宠物在寄养期内发生的任何疾病、突发性病变及其导致的后果承担任何责任和费用。3.请确认您的爱宠没有攻击性，如果它们对人、他人的财物或其它的宠物造成损害，您将会是全部责任的承担者。4.我们不对12个月以下及7周岁以上的狗和猫在寄养期内发生的任何突发性状况及意外事故和死亡承担任何责任。5.在您的宠物被寄养之前，一个常规的

2、健康检查是必须的。在任何情况下，您将承担全部的检查费用。（例：宠物在入住前被检查出已感染疾病或携带病毒，被谢绝入住。当然，您预付的寄养费用将被全额退还。）检查费用为：人民币59元/狗/次 (细小及犬瘟)，人民币79元/猫/次（猫瘟及猫流感）。6.在您的预定寄养期内，我们负责它们的住宿，饮食，清洁卫生。如果您没有特殊的要求，我们会为您的宠物提供一日两餐及三次20分钟的户外步行（仅限于狗，雨雪天气除外）。7.您可以自带您的宠物钟爱的食物，但您将不会因此而得到任何价格上的折扣。8.我们不承担任何义务和责任在您的宠物结束寄养之后。9.我们有权利处置超出寄养协议期限3天的宠物，并不再另行通知它的主人。1

3、0.我们以公元历的自然日来计算入住天数，20:30后入住，我们将从下一个自然日开始计费。11.请在您的宠物入住前付清全部的寄养费用。您将不能得到任何形式的退款哪怕因为您的宠物没有入住、提前离开或延期入住。12.任何未全额付清全部的寄养费用的、提前的、延后的预约单都将被视为无效。13.请确认您填写的联系人及联系方式真实有效。14.本协议以中文文本解释为准，我们不对因语言问题而产生的其它理解进行赞同。15.请签署您的姓名表示您已理解并接受上述条款。Dear customer,In order to give your pet a safe and healthy living environmen
